Description
|
In October 2002, the Effective Interventions
Unit published 'Integrated Care for Drug Users: Principles and Practice'.
The rationale behind integrated care is that many people with drug
problems have a range of needs and problems which require co-ordinated
interventions from a number of agencies and service providers. The
planning and delivery of appropriate and relevant treatment, care
and support requires a clear understanding of the nature and extent
of the needs and the attributes and aspirations of the individual.
The process of assessment is central to identifying those needs.
